At The Integrated Care Partnership, we believe that improving health goes beyond medical care. Through the Pulling Together Fund, we have invested in local community organisations that provide practical, preventative and wellbeing support for residents across Epsom & Ewell. By donating vital funding and working with trusted voluntary sector partners, we have helped people access the right support at the right time, reducing isolation, improving mental wellbeing and tackling the wider factors that affect health.
Supporting Older People to Stay Independent - Age Concern Epsom & Ewell
Funding has enabled Age Concern Epsom & Ewell to expand its outreach service, connecting with older residents who may otherwise remain isolated or unaware of available support. Working alongside GP practices, community venues and local partners, the service has provided personalised advice on benefits, housing, dementia support, transport, social activities and financial assistance. The outreach programme has strengthened links between health services and the voluntary sector, helping older people remain independent and connected within their communities.
Helping People Facing Poverty Before Crisis - Good Company
Our funding has also supported Good Company, whose work focuses on preventing hardship as well as responding to it. Through Advice Cafés, community support, food provision and partnership working, thousands of local residents have received practical advice, financial guidance and ongoing support. The organisation continues to develop coordinated referral pathways between health services and community organisations, ensuring patients can access the help they need quickly and efficiently while reducing reliance on crisis support.
Supporting Mental Health and Winter Wellbeing - Love Me Love My Mind
Working with Love Me Love My Mind, the Pulling Together Fund helped deliver the Winter Wellness Project for local residents experiencing financial hardship, poor mental health and social isolation. The project supported 26 households (over 78 people) by providing food vouchers, energy vouchers, winter warm packs, electric blankets and access to a welcoming community space with peer support and wellbeing activities.
The project demonstrated that relatively small interventions can prevent people reaching crisis point while strengthening community resilience and improving wellbeing.
Digital inclusion service – Tech Angels
Our contribution to Tech Angels supported digital inclusion within our local community. By helping residents access devices, connectivity, and digital skills, the initiative enabled more patients to engage with NHS digital services, including online appointment booking, repeat prescription ordering, the NHS App, and virtual consultations. This is particularly beneficial for older adults, people with long-term conditions, those on low incomes, and individuals at risk of digital exclusion, helping to reduce health inequalities and improve access to care.
Tech Angels also attend the ICP’s NHS App Café’s to be on hand to support our patients.
Supporting Families Through Serious Childhood Illness – Momentum
The Pulling Together Fund has also supported Momentum Children's Charity, helping families in our local community facing the devastating impact of childhood cancer and other life-limiting conditions. Working alongside NHS hospitals, Momentum provides practical, emotional and therapeutic support to children, parents and siblings throughout diagnosis, treatment and, where needed, bereavement.

Strengthening Community Partnerships
The Pulling Together Fund has enabled our surgery to take a more collaborative approach to supporting patients by investing directly in local organisations that have a deep understanding of our community's needs. Through these partnerships, we are able to address the wider determinants of health—such as loneliness, financial hardship, social isolation, and mental wellbeing—alongside the clinical care we provide. By connecting patients with trusted community services, we are helping to improve health outcomes and build a healthier, more resilient community across Epsom & Ewell.
The Fund has also strengthened partnerships between our surgery, the Integrated Care Partnership, and local voluntary and community organisations. These relationships have created a strong foundation for ongoing collaboration, enabling us to develop innovative projects that continue to improve the health and wellbeing of our patients and people across Epsom & Ewell.
Collectively, these initiatives demonstrate the value of investing in community-based support. By providing earlier intervention and addressing issues before they escalate, they help improve people's quality of life, reduce demand on NHS services, and ensure patients receive holistic support that extends beyond the consulting room.
