Book An Urgent Appointment
Urgent appointments are limited.
When booking, please provide a detailed reason for your request. These appointments are conducted over the phone.
The clinician will call you at some point during the morning surgery hours.
We will make every effort to ensure you are seen the same day. If the doctor determines that an in-person visit is necessary, or if you prefer a face-to-face appointment, we will invite you to come in.

Filling out your Accurx online form carefully helps our Triage Team understand your request and get you the right help faster. Here are some quick tips to make sure your form is clear and complete:
Watch a Short Demo

Before You Click Submit
- Be Clear and Specific - Tell us what’s wrong, when it started, and how it’s changed. Include anything that makes it better or worse.
- Stick to One Issue per Form - If you have another issue, submit a separate form (unless it’s clearly linked)
- Double-Check Your Contact Details
Need Help Filling in the Form?
You Can Use Your Voice
If reading or writing is difficult, your phone can help using voice typing:
- Open the Accurx form on your phone.
- Tap the microphone icon on your keyboard when you reach a text box
- Speak your answers out loud — your phone will type what you say. Example: “I have back pain for three days. It is getting worse.”
You can also use Siri or Google Assistant to translate into English:
- Example: “Translate to English: I have pain in my knee for one week.”
- The translation can then be copied or spoken into the form.
If You Can’t Speak or Read English Well
- Ask someone you trust (friend, family member, or carer) to help you fill it in.
- Use a translation tool (like Google Translate) to read or write your answers.
- Call the surgery if you need help — our Care Navigators can assist you in completing the online form.
- Tell us if you need an interpreter — the NHS can provide free language support.

Sickness Certificates (Fit Notes)
You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.
Home Visits
Please be aware that our GPs will always call to assess the home visit request first.
If you need a home visit, you can help us by calling reception before 11am.

Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only arrange a visit for you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls
You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby.

Life Threatening
Call 999 or go to A&E now if:
- you or someone you know needs immediate help

- You have an urgent medical issue requiring on the day attention
Non-urgent
Use NHS 111 if:
- You need help now, but it’s not an emergency
There will be someone to provide you with advice and to direct you to a clinician if it is necessary.
